Mistakes happen. They're part of life, and they're part of poker.  But, after a recent viral mistake at the WSOP-C Baltimore Main Event where the pot was pushed to the losing hand, plenty of questions were raised. 

Veteran tournament director Matt Savage has already weighed in on 'Savage Rules', and we've talked to from both players in the hand. We've now gathered an all-star team with expansive experience and knowledge of the industry to dig into the issues. 

What can each of us do to reduce major, tournament-altering mistakes? What is expected or wanted of staff and players? Should media members or onlookers speak up when they see something?
