What’s the most controversial poker ruling you’ve seen?

It’s Day 2 of the WSOP Main Event and we are approaching the last level of the day, leaving around 40% or so of the field remaining. It’s still a full day away from the money bubble, but the tournament is starting to feel more real, especially for the recreational and bucket list players,  

This table has a mix of professionals and casual players, and the action has been fast and furious for most of the day.The hand in question begins with the first two players to act folding their hands and the third player to act opening for a raise…. The dealer had not yet grabbed the two folded hands from the initial players, so they are randomly in the middle of the table instead of creating a muck in front of the dealer. 

The big blind, who was buried in his phone, looks up and sees a card near him so he grabs it. Moments later, he realizes he has 3 cards so puts what he thinks is the card that he grabbed back where it was. Players notice this and eventually the floor is called.

After the dealer explains the situation, the floor goes through a lengthy thought process. He pulls the first player that folded to the side and asks what his cards were. Afterward, he does the same for the second player who folded. 

Then the floor looks at the four cards still in the middle of the table, mucks two of them, and says this is the second player's folded hand. The first player's hand, however, is wrong. 

The floor pulls him aside again, then goes to check the big blind's cards. He takes one card from the big blind, replaces it with one card from the middle, and then mucks the middle cards, saying that now the big blind has the proper hand.

Everyone’s a little confused, but nonetheless the hand continues. The raise had already happened and a player to his left calls, while the rest folded around to the big blind… who three-bets (becsause, of course he does). The opener folds and the caller back-jams 20 blinds with pocket nines. The big blind calls and tables aces. A clean runout eliminates Mr. Nines from the tournament, and needless to say he was NOT happy.
